Online Forms

At West Florida Optometry, we offer HIPAA COMPLIANT online forms that can be completed with a phone, tablet, or computer. Forms do not need to be printed. A text message or email will be sent with a link to complete these forms two days prior to appointment. Another reminder will be sent one day before when forms are not completed. We encourage our patients to complete forms prior to appointment for efficiency. Please come 15 minutes early if forms are not completed at time of appointment.
